Under the supervision of the CHI Health at Home agency Director, the Hospice Chaplain is responsible for providing spiritual/bereavement care and counseling to Hospice patients, families, staff and volunteers. **Essential Key Job Responsibilities** _1. Documents assessment, planning and coordination of spiritual care of the patient/family:_ _2. Meets Spiritual care needs of patient and family_ _3. Coordinates agency Bereavement Services:_ _4.
