💼 The Opportunity

We’re hiring a Clinical Liaison (RN) to support the launch and growth of a new hospital-partnered service line in Philadelphia . You'll act as the face of myLaurel inside the hospital—building trust, identifying eligible patients, and promoting our home-based acute care programs .

This is a hybrid clinical-strategic role perfect for a nurse who excels in care coordination, patient education, and stakeholder engagement.

🧩 Key Responsibilities

🤝 Hospital Engagement – Build strong relationships with care teams, physicians, and hospital leadership
📈 Referral Management – Identify eligible patients and guide them into myLaurel’s acute care programs
💬 Patient Education – Clearly explain services and care plans to patients and caregivers
⚙️ Operational Integration – Seamlessly embed myLaurel into hospital discharge and care workflows
🎯 Strategic Growth – Collaborate with internal teams to drive program adoption
📊 Track Results – Monitor key performance metrics including referrals, patient satisfaction, and outcomes
🤝 Cross-Team Collaboration – Coordinate with myLaurel’s internal care and operations teams

📅 A Day in the Life

  • Attend hospital huddles and rounds

  • Conduct chart reviews and referral assessments

  • Deliver program education to patients, caregivers, and staff

  • Track referral data and outcomes

  • Serve as a clinical ambassador for home-based acute-level care

What You’ll Need

🩺 Active, unrestricted RN license
🏥 5+ years of clinical experience (hospital inpatient or ER preferred)
🧠 Expertise in care transitions , discharge planning, and readmission prevention
📢 Strong communication and relationship-building skills
💻 Proficient in using EHRs and navigating hospital systems
🎓 BSN or related degree preferred
🚀 Bonus: Experience working in program development or liaison roles
