Job Description

Hospice of the East Bay (HEB) is a community-based nonprofit organization providing compassionate home care to terminally ill patients and their families. Care is delivered by a dedicated interdisciplinary team, and bereavement support is offered to families for one year following the death of a loved one. Hospice services are provided regardless of ability to pay, supported through insurance billing and community donations. Hospice East Bay has been certified the past five years in a row as a "Great Place to Work".

Coverage Areas: American Canyon, Hercules, Richmond, Oakland, Castro Valley, Walnut Creek, Pleasanton, Antioch, Brentwood and the surrounding cities in the area.

Position Summary:

Reporting to the Interdisciplinary Team (IDT) Manager, the Revisit Nurse provides skilled hospice nursing care to patients in their homes. This role involves assessing and coordinating patient and family care needs in collaboration with the attending physician, Medical Director, Nurse Case Managers, and other members of the interdisciplinary team.

Essential Functions:
  • Provide direct hospice nursing care to patients and families in the home setting
  • Assess patients and communicate changes in condition, treatment response, and medication effects to the attending physician and interdisciplinary team
  • Coordinate patient care with Nurse Case Managers, other agencies, suppliers, and facilities involved in care
  • Supervise Home Health Aides in the home as needed
  • Maintain timely, accurate clinical documentation and reports in accordance with agency policy
  • Participate in nursing staff meetings, quality improvement activities, and staff training
  • Follow National Patient Safety Goals and agency hand-off communication processes
  • Coordinate time off with IDT Managers to ensure adequate staffing
  • Perform other duties as assigned
Position Requirements:
  • Demonstrated ability to assess and respond to the needs of patients and families in the home setting
  • Strong clinical nursing skills, including IV therapy
  • Ability to manage emotional stress and provide care with compassion, respect, and without judgment
  • Effective communication skills and ability to collaborate with interdisciplinary team members
  • Ability to work independently, problem-solve in crisis situations, and manage competing priorities
  • Demonstrated professionalism, dependability, initiative, and sound judgment
  • Clinical competency as observed during supervisory visits
Education and Experience:
  • Current California Registered Nurse (RN) license
  • Minimum of one year of nursing experience, preferably with terminally ill patients
  • Valid California Driver's License and reliable vehicle in good working order
  • CPR certification and IV skills required
  • Commitment to maintaining current knowledge of hospice standards of practice and pharmacology through continuing education
  • Hospice Nurse Certification preferred
